2008 ACHIEVEMENT AWARDS
NOMINATION FORM
Each year the Puyallup/Sumner Chamber celebrates and honors the accomplishments of its members by presenting five major awards at the Annual Banquet. Please use this form to nominate potential award recipients in any or all of the following five categories:
John Porter Memorial Community Service Award is designed to recognize exceptional leadership of a cause, program or project, which makes a significant impact on the community and reflects the spirit of community pride and progress.
Volunteer of the Year will be selected for exceptional leadership of a cause, program or project; excellence in commitment, quality and spirit of service to others and demonstrated initiative and originality in designing or creating an innovative project or benefit to meet community needs.
Ambassador of the Year will be awarded to one Chamber Ambassador who has demonstrated exceptional commitment and involvement in Chamber events and programs.
Small Business of the Year (1-30 employees).
Large Business of the Year (more than 31 employees)
To recognize and honor businesses that have demonstrated “Best Practices” of entrepreneurship such as, sound business planning, fiscal responsibility and work process innovation in operating their business
Criteria
· Any Chamber “for-profit business” member
· The applicant must be the owner, partner or major shareholder of the business, and active in its day to-day operations
· The business must be financially stable and in operation for a minimum of three years
· Companies must be a member of the Puyallup/Sumner Chamber of Commerce at time award is presented

· Unique and Innovative Approaches
· Community Involvement and Contribution
